To meet the growing complexity of educational teamwork and limited resource capacity, the 2025 Secondary Transition Leadership Institute has been restructured for maximum impact. This year’s format delivers targeted content, dedicated support, and expanded team planning time, ensuring your travel and time away from students are both meaningful and transformative.

  • Breakout Session Content:
    • Strategies for breaking barriers in the Secondary Transition process for Multilingual Learners
    • Annual goal writing workshop for targeting essential skills for a successful transition to adulthood
    • IN! Inclusive Higher Education (18–21) and Concurrent Enrollment opportunities for students with Intellectual Disabilities
    • Instructing and supporting student independence with a  K-12 Timeline  from the Colorado Community College System
    • Agency resource mapping & implementation for interagency team development and planning
    • 18–21 Secondary Transition planning and programming
  • Working Lunch – Student Engagement through Student-Led IEPs
  • Team Goal Planning

Day 2 – 8:00 AM to 3:00 PM

  • Interactive Opening – Person-Centered Planning
  • Agency Introductions – Roles in Transition & Braided Services
  • CDE Team Content Expert Support – Technical assistance and resource opportunities
  • Working Lunch – Career and Technical Education (CTE) Models for Access & Success for student with disabilities
  • Team Goal Planning & Collaboration Hub with agency and content expert support
